Youth Enrollment Step-by-Step Guide

  • 1. Orientation Video

    • Please watch the Orientation Video and fill out the form it gives you after watching the whole thing.
  • 2. Complete the Intake Meeting

    • Please schedule your Intake Meeting through the Bookings page under "Youth Intake Meeting".
      • - Your parent or guardian must be present as we will need to witness both you and your
        parent/guardian signing forms
      • - You must fill out the UASIS online registration form during this meeting.
        • - Please make sure to include previous high schools so that we can recieve your transcript(s).
      • - You will sign the Release Waiver, Grievance form, & code of conduct form during this meeting.
      • - You will need to bring your GED/Adult Education Eligibility Form with all the nessessary signatures to
        the meeting.
      • - If you have a Disability or require reasonable accommodations we will discuss those in this meeting.
      • - We will have you schedule your CASAS placement Testing by going to this link and scheduling it in the
        box labled "CASAS eTest"
        • - You will upload your photo ID during this scheduling process.
        • - Your fees must be paid before taking your CASAS placement eTests.
  • 3. Pay the Fees

  • 4. Complete CASAS Placement Tests

    • - Complete the CASAS Reading eTest
    • - Complete the CASAS Mathematics eTest
  • 5. Recieve Your Transcript(s)

    • We must recieve any of your transcript(s) before continuing past this point.
  • 6. Complete the Review & Planning Meeting

    • Please schedule your Review & Planning Meeting through the Bookings page under "Review & Planning Meeting".
      • - Parents/Guardians are welcome for this meeting, though not required.
      • - You and the Instructor will discuss Goals, Plans, and Barriers for your education.
      • - We will schedule your set start date.
  • 7. Complete your 12 Hours

    • Before you are officially enrolled you will need to attend a cumulative total of 12 hours in the program.
      • - The orientation process, placement tests, and meetings all count toward this time.
      • - You will be able to start on classes before your 12 hours is reached and you are officially enrolled.
        • - This is how you will finish the rest of your 12 hours.
  • Once you have completed these steps you will be officially enrolled in
    Utah Adult Education Online Academy
